Breast Augmentation with Fat, an alternative approach


Liposuction and simultaneous Breast Augmentation – Don’t waste that fat!


The female breast is made up of breast tissue and fat, so the most natural material for breast augmentation would be your own fat. Plastic surgeons have understood this for some time; but concerns about the ability of grafted fat to survive as well as other concerns related to effects on detection of breast cancer, have made surgeons proceed with caution.
Over the last few years improved fat grafting techniques, have resulted in significantly improved results as well as alleviated previous concerns regarding the detection of breast cancer.
It is now possible to provide predictable results using your own fat for breast augmentation for small to moderate size breast augmentation such as a B or C cup, or larger if you already have moderate size breasts.
Previously, fat grafting for breast augmentation would usually require multiple procedures in order to get the desired breast size; but now by preparing the breast with a simple tissue expansion device prior to the fat grafting procedure, it is possible to obtain excellent results with just one fat grafting procedure.
As we obtain a better understanding of fat grafting, we are also understanding that fat is a rich source of stem cells with the ability to enhance healing and facilitate the restoration of normal tissue when tissue has been injured such as after radiation for breast cancer. So it is now possible to perform fat grafting for breast reconstruction after mastectomy for breast cancer, as well.
Fat grafting has been used to treat problems with breast implants such as implant rippling and capsular contracture.
At the Bougainvillea Clinique we now offer breast augmentation using your own fat, combining liposuction and fat grafting with the Brava® device to obtain predictable and extremely natural results. Of course fat grafting eliminates any risks related to breast implant procedures such as loss of nipple sensation, capsular contracture or hardening, or implant deflation. Fat grafting can also be used to cover saline implant rippling.
An added advantage of course is that on can now enjoy the benefit of liposuction to reduce the unwanted bulges around your stomach and thighs, and at the same procedure use the fat removed to enhance your breasts. Using meticulous liposuction techniques and newly developed instruments to gently harvest and prepare the fat, the procedure can now be done much more economically than before, with minimal discomfort, rapid recovery, and high predictability of fat survival.
This procedure is not for everybody, particularly very slim women who want fairly large breasts, but it is ideal for women with moderate size breasts who have lost volume after pregnancy who want their youthful fullness restored, while at the same time flattening those unwanted bulges that have developed over the years. It is also ideal for younger women who only want a moderate enlargement, but want to avoid the use of breast implants. For the best and most predictable results, the individual must also be prepared to use the Brava® system for a few weeks prior to the grafting procedure and for a week after the procedure.

Cohesive Gel Implant Update


Mentor Corporation, the makers of gel and saline implants, recently selected me as one of five plastic surgeons from the United States to attend a comprehensive training program in Stockholm, Sweden.
The training program was titled ' How to run your best Breast Practice' and was conducted at the magnificent Victoria Kliniken, by the world renowned plastic surgeon Dr. Charles Randquist. The main objective and emphasis of the program was to train us in the planning and surgical technique to place the new highly cohesive gel implants, popularly known as 'gummy bear ' breast implants. These implants known as the Mentor CPG gel breast implants will hopefully be cleared soon for use in the USA by the FDA. Highly cohesive gel implants have been used for more than 10 years in Europe with excellent results, and Dr. Randquist is one of the most experienced surgeons worldwide in the use of these implants. Studies show that these CPG cohesive gel implants have a number of advantages over presently available implants, including less rippling, more stable breast shape over time, less implant failure over a 10 year period, and possibly lower capsular contracture or hardening rate. Because of the anatomic shape of these implants, these implants provide a more natural shape breast for many woman.
These implants will not necessarily be the best choice for all women, as they are a little firmer to the touch and do not move as naturally as the softer less cohesive gel implants that are presently available in the United States.
In addition to the training provided in these new implants, Dr. Randquist shared with us some of his secrets in running a highly successful breast augmentation practice, including techniques that result in much faster recovery with little or no pain, and improved patient satisfaction. Many of these concepts I have already used in my practice for some time, but of course I continue to learn and incorporate new ideas into my practice to help improve my patient experience. Dr. Randquist's studies have confirmed that many of the techniques and protocols used at the Bougainvillea Clinique already, result in significantly faster recovery with little or no pain, and the ability to return to essentially normal activity within just a few days, sometimes only a day or two.
I look forward to the release of the new highly cohesive 'gummy bear' style implants so that I can be one of the first to offer this option to my patients.

Deciding on breast implant size

One of the most perplexing decisions for prospective breast augmentation patients is deciding on breast implant size. There really is no absolutely right or wrong answer to this question, but it depends on two things; how big you as a patient want to be, and the recommended maximum size implant that will fut your chest and body type.
At the Bougainvillea Clinique, during your consultation, one of our first questions is 'What cup size would you LIKE to be?' The answer will then guide us in helping you choose the right implant. It is important to stress that we can never guarantee an actual cup size, as this is of course dependent on many factors, including how tight you like to wear your bras, the brand of bra etc. But this is a good starting point for us to assess whether you would like a small, moderate or very large augmentation.
The next step is for me to assess your body type, present breast size, chest dimensions and skin and tissue thickness. Combining this assessment with what your desires are, I can then make an assessment as to the volume and dimensions of the implant needed to meet your expectations. Frequently however, patients would like to be larger than I would recommend, and I will then caution the individual that implants that are too large are more likely to cause problems in the future such as sagging breasts, stretched skin etc.
The next step at the consultation is to capture an image with our Vectra 3-D Breast Sculptor imaging system. At your preoperative visit we then use this image to create 3-D predictions of the final breast appearance using implants of varying sizes Other traditional methods such as placing implants in a bra and looking at pictures are also used to help you decide which implant size to choose.
It is our goal to provide you with the look you desire while still staying within the boundaries of choosing an implant that is not too large for your body type. Of course the final decision we leave up to you, but remember you as the patient have to live with the consequences of breasts that are too large, so our most frequent request is do not go too big!
